If you are completing this form, you are the Team Captain.
Team Captain is responsible for collecting payment and fees from your team. This is not the responsibility of tournament organizers.
To register you’ll need 4 players.
If you don’t have a team, email mainstreetbusinesses@gmail.com and we will do our best to match you with one!
Golf Carts are included in the fee. If you choose not to use a cart, the fees are the same.
